DIRECTIONS TO THE WARSAW LOCATION US 30 to East Center Street. Turn north on E. Center Street extended (away from downtown) into the Center Centre. The clinic is on the near end of the building with a brick entry. We are across the parking lot from Bob Evan’s.

DIRECTIONS TO THE FW SOUTHWEST LOCATION From I-69, take exit 102. Go WEST on US 24 (Jefferson Blvd). Turn right at the stoplight between Bob Evans and the BP gas station onto Coventry Lane. Proceed straight through the next traffic light. Scott’s grocery store is on your right. Take the next entrance on the left. Then take a sharp left into our parking lot. We are next to Star Financial Bank.
